Field Test and Repair Spec for_________________________________________________

Stucco Retro West TN, Inc. appreciates that you have chosen us to perform the following serviceson the above referenced.

The Field Test as outlined on page 11 of the Stucco Retro Program.

Included with the Field Test is an opinion letter on the condition of the home and a Repair instruction document that identifies in general the required repairs and the areas of specific repairs. The cost of the Field Test is $_____________.

The Retro Spec. The Retro Spec can only be written after the Field Test is completed. The Retro Spec includes
the delineated outline for repairs as shown on pages 12 and 13 of the program.

Included with the Retro Spec are the required repair details, a written budget letter, a list of preferred contractors, a bid form to use to obtain quotes and a quantity survey of the work to be done. Please note this survey is not
guaranteed for accuracy. Further, any budget provided is only an estimate and is not a bid or a guarantee. Actual costs may vary widely from the budget due to the availability of labor and materials and for other reasons.
Each contractor should do their own survey. Failure to visit the site or make field measures does not relieve him
from the intended scope of work. The cost of the Retro Spec is $_______________.

One set of the above listed documents will be delivered in a loose-leaf binder with the executed agreements
included. This delivery will be made by appointment and we will be available for approximately one hour to review your report and answer your questions.

Additional sets of the bid documents can be obtained for $_________per set.

Any additional time spent in consultation shall be billed at our professional rate of $110.00 per hour unless covered
by contract administration. Additional office work shall be billed at $32.50 per hour plus supplies.

A 50% deposit is due on signing of the agreement with the balance due on delivery. This quote is good for 60 days.
